We're hiring again and this time we are on the hunt for a fabulous, dedicated and compassionate Support Coordinator with experience in this field. The right candidate would be working with an incredibly tight knit and supportive team and some of the roles and responsibilities will include:
Support the participant to implement their NDIS plan, including assistance to identify and select providers
Monitoring plan outcomes & plan expenditure to ensure the plan remains within budget
Strengthen and enhance the participant’s abilities to self-direct their own supports and participate in the community;
Ensure mainstream services meet their obligations (i.e. housing, education, justice, health)
Ensure clear, accurate and timely record keeping including making information accessible to the participant
Capture any required data for the organisation and the Agency Supporting individuals to access the NDIS
